If you are an undergraduate student who will only have a few weeks to conduct your summer research, take the cover sheet for the Request to Amend an Approved Protocol with the appropriate signatures when you leave the U.S. If you need to make changes, you can fax the cover sheet with the amended protocol to the IRB^* staff. The staff will consult with your advisor to make sure she or he approves the changes and will respond immediately to your request.
